Abstract
A 64-year-old male who had an intermittent right shoulder pain with a mild LOM for 11 years revealed a bizarre acro-mial bone spur that antero-inferiorly directed and made an osseous connection to the coracoacromial ligament to near-ly coracoid insertion site. After failure of conservative treat-ment, arthroscopic excison of a bizarre keel spur was per-formed and the pain free motion was achieved.
